棘阿米巴原虫相关研究 棘阿米巴角膜炎是由棘阿米巴原虫造成的角膜损伤疾病,临床中发现大部分的棘阿米巴角膜炎患者多长期配戴隐形眼镜。棘阿米巴角膜炎会造成的症状包含:眼睛畏光、视力模糊、眼睛异物感以及失明等,感染严重者需进行摘除眼球手术。隐形眼镜配戴除棘阿米巴角膜炎外,细菌性感染也时常发生,细菌性角膜炎使用抗生素作為主要治疗手段,但常用的眼用抗生素对於棘阿米巴并不具杀灭效果,而棘阿米巴有携带致病性细菌的能力并造成严重角膜炎,临床案例也指出多重感染性角膜炎较难被治癒且严重性较高。过去研究已经发现棘阿米巴具有多种药物耐受相关基因,而在我们的藉由细菌表达系统以及抗生素敏感性实验,我们证实棘阿米巴中的β-内酰胺酶对於数种β-内酰胺类抗生素具有干扰效果。除了抗药性相关基因导致棘阿米巴相关角膜炎治疗效果不佳之外,临床案例报告指出葡萄糖酸氯己定(chlorhexidine digluconate, CHG) 虽然為治疗棘阿米巴角膜炎的第一线药物,然而在部分案例中发现 CHG 无法有效治癒棘阿米巴相关角膜炎。因此,在本研究中我们想要了解為何体外试验证实CHG有良好的杀灭棘阿米巴效果而在临床使用却无法有效控制棘阿米巴感染。我们的研究成果将提供棘阿米巴用药的参考,并提升临床药物对於棘阿米巴感染症治疗的效果。 口腔鞭毛虫相关研究 口腔鞭毛虫是一种有鞭毛的厌氧原生动物,被认為是人类口腔中的共生生物,主要存在於牙周袋、牙齦缝隙和牙菌斑内。研究显示它与口腔卫生不良和牙周炎有关。儘管先前的研究表明口腔鞭毛虫会引起细胞损伤併吞噬宿主上皮细胞,但其对牙齦细胞的病理作用仍不清楚。而且,在患有肺部疾病的个体的支气管肺泡灌洗液(Bronchoalveolar lavage fluid, BALF)中也检测到了它的存在。因此,我们的首要目的是确定口腔鞭毛虫对牙齦和肺细胞系的细胞毒性和免疫作用。我们的结果证实,口腔鞭毛虫可以引起牙齦细胞的细胞毒性,破坏细胞连接,并诱导牙齦和肺细胞系产生介白素-6。 接下来我们想要解析口腔鞭毛虫在肺炎中的潜在作用,因此阐明寄生虫与疾病之间的关係十分重要。我们与嘉义基督教医院胸腔科合作採用巢式聚合酶鍊式反应检测肺炎相关病患的BALF,并分析肺炎患者中与口腔鞭毛虫感染相关的临床因素。研究中纳入了2022年8月至 2023 年8月期间在成大医院接受肺支气管镜检查的61名肺炎患者。我们的数据显示,口腔鞭毛虫的检出与细菌感染、高肺炎严重程度指数(PSI)评分、鼻胃管餵食和肺部併发症之间存在显着关联。逻辑迴归分析也显示,口腔鞭毛虫与肺炎患者的这些临床因素之间有强烈的相关性。这些结果显示肺炎中的口腔鞭毛虫感染伴随细菌感染和严重的临床表现。 除了肺炎之外,我们也与成功大学附设医院胸腔科合作检测口腔鞭毛虫於肺癌患者的流行率。肺癌是全世界最常见的恶性肿瘤之一。我们的研究结果进一步显示口腔鞭毛虫的感染导致致癌相关的路径的显着上调,其中包括细胞增殖、迁移和抗药性基因的表达。 |
