?、為提升職場健康，依據職業安全衛?法及勞?健康保護規則，特約林??庚醫院家醫科及職業 醫學科主治醫師，針對教職員臨校進?健康服務。

?、临校健康服务并非?诊，不会开立处?亦不需健保卡，提供15词30分鐘免费专业諮询，并採预约制，省去排队等待的时间。

叁、為尊重同仁个?隐私，将安排可维护隐私的空间，进??对?健康諮询。

四、非?作影响??状况之諮询(私?问题)将不留下任何?谈纪录，请各位同仁放?。

五、諮詢服務內容：健康報告解釋、就醫科別建議、個?疾病諮詢及慢性病衛教、職業傷病診療與 防治、?作場所環境改善及防護具選擇與建議、職場傷病復?及配?、職業性壓?及過勞等?作相關之傷害與疾病。

1. To enhance workplace health, in accordance with the Occupational Safety and Health Act and Labor Health Protection Regulations, specialists from the Family Medicine and Occupational Medicine Departments of Linkou Chang Gung Memorial Hospital provide health services for faculty and staff on-site.

2. On-site health services are not outpatient services; no prescriptions or health insurance cards are required. They offer free 15-30 minute professional consultations by appointment, eliminating the need for queueing.

3. To respect colleagues' privacy, private spaces ensuring confidentiality will be arranged for one-on-one health consultations.

4. Consultations unrelated to work-related physical or mental conditions (personal issues) will not be documented in any interview records. Colleagues can rest assured.

5. Consultation Services: Interpretation of health reports, recommendations for medicalspecialties, individual disease consultation and chronic disease education, diagnosis and treatment of occupational injuries and diseases, workplace environmental improvement and selection and recommendations of protective equipment, return-to-work and job allocation for workplace injuries and illnesses, occupational stress and overwork, and other work-related injuries and diseases.

6. Appointment Procedure: Please complete the reservation form at least two days prior to the physician’s on-site visit. The Environmental Safety Office will coordinate the scheduling.
